If your child is five years old, it is time for
kindergarten. Have you registered yet? To be better prepared gather
all the required documents and be ready to share your child's
excitement. The following checklist will assist you.
Checklist
__ Register your child as soon as possible
__ Call the school district office or visit its
website
__ Check on summer hours for school offices
__ Find out if your child's fifth birthday is
before the official cut off
__ Find out if your child needs to be ready at
registration
__ Most school districts require the following:
- Birth certificate or similar proof of age
- Child's social security number
- Proof of address
- Health form
- Custody agreement, if applicable
- Proof of vaccinations
__ Most states require the following vaccinations
- DPT or DtaP
- Polio
- MMR
- Hepatitis B
- If your child is not vaccinated for any of the
above for a good medical reason, provide a doctor's letter explaining
why/
__ Answer your child's questions about kindergarten
__ Enjoy your child's excitement and prepare
yourself for this new adventure!
